Provides direct patient care within centre Standard Operating Procedure (SOP).
Demonstrates knowledge and clinical assessment skills appropriate to clinical area.
Documents nursing interactions in patient’s clinical record and ensures confidentiality.
Promotes continuity of patient care through verbal and written communication.
Assess, evaluates and implement individual patient care plan.
Takes direct and appropriate action in urgent and emergency situations.
Completes assigned responsibilities within established organizational, departmental and divisional specific guidelines, policies and procedures, standards and protocols.
Demonstrates effective communication and collaboration skills when interacting with patients/families, colleagues, other health care team members and staffs of other departments.
**Administration**:
Provides a leadership and resource role in the provision of patient care.
Acts as a unit management resource e.g. charge nurse, team leader as assigned.
To liaise /work with peers, members of the team to provide continuity of patient care and unit activities.
Addresses issues using the problem solving process and utilizes the nursing chain of command appropriately.
Liaise with other departments / Hospital Sultanah Bahiyah to ensure continuity of care for patients. Assist in collection of data for NRR and marking of patients’ attendances, recording of laboratory test results, serology status of patients and prepare for Visiting Consultant Nephrologist’s clinic visit.
**Education & Research**:
Participates in teaching and mentorship of graduate nurses and new staff.
Works with members of health care team in activities designed to improve patient care.
Acts as a preceptor and or resource for new employees, trainees and other staff.
Work with all health care team members in activities designed for the improvement of patient care outcome.
Assumes responsibility for own professional growth, identifying those areas of clinical knowledge and skills that need further development.
**Quality**:
Anticipates and acts to maintain a safe and healthy working environment.
Monitors effectiveness of standards of patient care and nursing policies and procedures, reports deficiencies to Head Nurse in a timely manner.
Participates in unit based quality initiatives.
Users research/current literature in the provision/supervision of evidence-based practice.
**Professionalism**:
Serves as a professional role model and resource person to junior nursing staff/dialysis assistant.
Demonstrates respect, compassion and cultural sensitivity to patients / families and other employees.
Advocates for excellence in patient care and for patient rights.
Assumes respoPost Basnsibility for own decisions, actions and behaviors.
Adheres to the national code of professional conduct for nurses.
Demonstrates initiative in the performance of duties.
Must have Post Basic Certificate.
