**PHARMACIST**:

- PHARMACY & SUPPLIES DEPARTMENT _

**Responsibilities
- Provide efficient pharmacy services in the Outpatient, Inpatient, Clinical & Training,

and Pharmaceutical & Sterile Preparation Units
- Major duties involve supply of medications to outpatients, inpatients and patient

care areas
- Provide pharmaceutical care in Medication Reconciliation areas, INR Clinic and

Multidisciplinary Clinic/s
- Provide inpatient counselling with accurate medication information and advice to

ensure optimum patient outcomes
- Participate in educational and training activities for professional development
- Participate in all departmental & organizational-wide quality improvement activities
- Perform inventory control & monitoring
- Perform other pharmacy-related duties assigned by the supervisor**
**Requirements
- Degree in Pharmacy (recognized by Malaysian Pharmacy Board)
- Registered with Malaysian Pharmacy Board
- Proficient knowledge of pharmacy practice, pharmacy inventory system & other

procedures related to drug supply
- Proficient working knowledge of Health Information System / Pharmacy software

& Microsoft Office
- Strong leadership capabilities to schedule & monitor subordinates’ work
- Team player with good interpersonal and communication skills (verbal & written)
- At least one (1) year experience in hospital pharmacy is preferable
