Pharmacist

**Responsibilities**:

- 1 Practices within the expressed limits of established policies and procedures, securing approval from the Head of
- Pharmacy for any deviations from the policies and procedures.
- 2 Ensures the provision of products with optimum quality by following Standard Operating Procedures (SOP) and quality
- assurance testing (if required).
- 3 Liaises with pharmacists or supervisors in other units and Head of Pharmacy in day-to-day issues. Reports to the
- Head of Pharmacy on a regular basis.
- 4 Reviews prescription orders and ensures that medications prescribed are safe and appropriate for intended use. This
- includes potential drug side effects, harmful interactions with other drugs and the suitability of treatments for patient
- with particular health conditions.
- 5 Verification of EMR to ensure that medications prescribed are safe and appropriate for intended use.
- 6 Uses knowledge to advise patients about the safe and appropriate use of medicines they have been prescribed with. Provides counselling on medication/medical device use to patient e.g. method of administration/use, effects, dosage
- regimen, particularly those who require complex drug therapy as well as provides bedside dispensing and counselling.
- 7 Works collaboratively with other healthcare professionals to devise the most appropriate drug treatments for patients.
- 8 Liaises with physicians, nurses and other healthcare professionals to ensure the delivery of safe and effective drug
- treatment.
- 9 Contacts physicians to clarify any discrepancies or issues that appear in the prescription.
- 10 Responds to medication-related queries from within the hospital, other hospitals and the general public.
- 11 Supervises the execution of duties by staff within the unit that you are in charge of e.g. Outpatient pharmacy, Inpatient
- pharmacy or Cytotoxic Drug Reconstitution (CDR) and ensures that high professional standards are maintained.
- 12 Manages the work of the team/unit members to ensure optimum use of manpower
- 13 Formulates and supervises the compounding of extemporaneous medicines and lotions and pre-packing of medicines. To review and amend extemporaneous drug formulary and lotion formulary as required on a regular basis.
- 14 Participates in training or teaching of pharmacy staff, trainees and other healthcare professionals as required by
- management (internal and external pharmacy related CPD programs).
- 15 Maintains records on transactions of all controlled drugs (i.e. Psychotropic Drugs & Dangerous Drug) as required by
- Malaysian laws and regulations.
- 16 Ensures that medicinal products are stored and transported appropriately accordingly to the manufacturer’s
- recommendations to ensure freshness and potency.
- 17 Ensures the medications reach the patient in the correct form and dose.
- 18 Monitors consumption of stocks in the pharmacy, wards and units and takes appropriate measures to ensure cost
- efficient use of stocks and minimize wastage.
- 19 Strictly maintains patient confidentiality.
- 20 Promotes and maintains quality in all activities and ensures continuous quality improvement measures are followed.
- 21 Complies with and ensures that those directly supervised comply with department and hospital policies and
- procedures.
- 22 Participates in Pharmacy Department meetings, Pharmacy & Therapeutic Clinical Meeting, other department or
- hospital committee meetings and quality improvement activities.
- 23 Participates in the writing, review and amendment of department Policy and Standard Operating Procedures (SOP).
- 24 Maintains statistics and appropriate departmental documents and submits reports as requested by the Head of
- Pharmacy.
- 25 Assists the Head of Pharmacy in staff appraisal of staff under direct supervision (in consultation with the supervisor or
- Head of Pharmacy, if required).
- 26 Undertakes other relevant tasks as may be directed by the Head of Pharmacy from time to time.**Benefits**:
