**Responsibilities**:
**1. Patient Care and Assessment**:
- Provide direct patient care in a ward setting, ensuring the physical and emotional well-being of patients.
- Perform comprehensive assessments of patients, including vital signs monitoring, physical examinations, and evaluation of symptoms.
- Administer medications, treatments, and procedures as prescribed by healthcare professionals.
- Monitor and evaluate patient responses to interventions, noting any changes in condition and reporting to the healthcare team.
- Collaborate with the multidisciplinary team to develop and implement individualized care plans for patients.
**2. Nursing Documentation and Record-Keeping**:
- Maintain accurate and up-to-date documentation of patient assessments, interventions, and outcomes.
- Ensure proper documentation of medication administration, including dosage, time, and any relevant observations.
- Record and report any incidents, medication errors, or adverse events according to hospital policies.
- Document patient education and instructions provided to patients and their families
**3. Patient and Family Education**:
- Educate patients and their families on their conditions, treatment plans, and self-care techniques.
- Provide information and guidance on medications, dietary restrictions, and lifestyle modifications.
- Address patient and family concerns, answering questions and offering emotional support.
- Collaborate with other healthcare professionals to coordinate patient education efforts.
**4. Collaboration and Communication**:
- Communicate effectively with the healthcare team, including physicians, other nurses, and allied health professionals.
- Participate in interdisciplinary rounds and case conferences to discuss patient progress and plan of care.
- Provide accurate and timely handovers during shift changes, ensuring continuity of care.
- Collaborate with other departments and services to facilitate the smooth flow of patient care.
**5. Safety and Infection Control**:
- Adhere to hospital policies and protocols related to infection control and patient safety.
- Implement preventive measures to minimize the risk of healthcare-associated infections.
- Monitor patients for any signs of deterioration or safety risks, taking appropriate actions as needed.
- Maintain a clean and organized patient environment, ensuring equipment is properly cleaned and stored.
**6. Quality Improvement and Professional Development**:
- Participate in quality improvement initiatives, audits, and performance improvement activities to enhance patient care outcomes.
- Stay updated on current nursing practices, guidelines, and evidence-based interventions.
- Attend educational programs, workshops, and conferences to expand knowledge and skills.
- Engage in ongoing professional development to enhance nursing practice.
**Requirements**:
- Degree / Diploma / Bachelor
- Well-versed in technical skills, such as monitoring vital signs, checking pulse, and listening for indications of labored breathing.
- Identify and assist with conditions and symptoms that are sometimes complex.
- Freshies or 3 years of working experience in the related field.
- Excellent communication skills (written and oral)
