Job description: - Works collaboratively with other healthcare professionals to devise the most appropriate drug treatments for patients. - Liaises with physicians, nurses and other healthcare professionals to ensure the delivery of safe and effective drug treatment. - Contacts physicians to clarify any discrepancies or issues that appear in the prescription. - Responds to medication-related queries from within the hospital, other hospitals and the general public. - Formulates and supervises the compounding of extemporaneous medicines and lotions and pre-packing of medicines. To review and amend extemporaneous drug formulary and lotion formulary as required on a regular basis. Job requirements: - Bachelor Degree in Pharmacy (or above) - Registered with Malaysian Board of Pharmacy - Possess Poison License A and Annual Retention Certificate - 1-3 years of working experience - Able to be on-call (after office hour)
