: The designee will work as part of a team to provide a safe, effective, efficient care environment for patients, relatives and carers. They will be responsible for the assessment, planning, implementation and evaluation of programmes of care with direct supervision from senior staff. Responsible for the assessment of care needs, planning, implementation and evaluation of nursing care in multidiscipline ward / as assigned to. (Medical/Surgical/Paediatric/Gynae Ward) DUTIES & RESPONSIBILITIES: GENERAL 1. Carries out a full range of registered nurse duties, as a member of team and assisting other team members in assessment, planning, implementation and evaluation of the patient’s condition and care with support from the nurse in charge. 2. Ensures that own practices and interactions are adhering to Avisena and department policies based on the preservation of a client’s privacy, dignity and individuality and that confidentiality is maintained. 3. Participates in establishing a good working environment where clients receive a high standard of care. 4. Uses effective communication, written and verbal with all levels of staff including the multidisciplinary team, patients and relatives. 5. Maintains a safe environment when caring for those patients requiring frequent levels of nursing care. 6. Ensures all patient care and treatment are maintain in high standards and quality at all times. 7. Works with mínimal supervision once key learning outcomes of the induction/ orientation or preceptorship programme have been completed. 8. Is able to prioritize nursing care, recognize deterioration in a patient’s condition and alert senior staff immediately 9. Adheres to the agreed procedures as identified, and promotes by example, a high standard of good quality nursing care in the department. 10. Documents incidents in real time and escalated these to nurse in charge/ head of department. 11. Maintains complete, accurate, contemporaneous and legible written records of care. 12. Fosters good relationships with other departments, the multidisciplinary team and visitors to the unit. Helps to achieve and maintain morale for both staff and patients. 13. Participates in the health education and explanation of treatment to patients and relatives (in ward and during discharge communication). 14. Provides support and assistance to HOD in managing ward Quality Improvement Activity (QIA). PROFESSIONALISM 1. Adheres to Avisena ‘s uniform and dress code policy. 2. Responsibles as a trained nurse for the control and administration of drugs within the regulations outlined in Medication Administration Policy. 3. Participates in ward projects, enhancing the standards of care. 4. Assists in updating of Professional practice/procedures in line with current research. 5. Contributes to the learning environment conducive for junior nursing and other staff in training. 6. Assists in the implementation of hospital initiatives. 7. Maintains own personal profile and ensure they meet the requirements for revalidation (APC). ISOLATION REQUIREMENTS 1. Responsibles for the safety and health of oneself, co-workers and others that may be affected during work, including make self-declaration of health status. 2. Practices of preventing and controlling of Hospital Acquired Infection by adhere to Hospital Infection Control SOP and guidelines. 3. Responsibles for own self’s health and comply with health status SOP as stated by Safety Health Environment Department. 4. Demonstrates clinical assessment and care treatment skills as per KKM guidelines. 5. Provides health education on awareness of infection prevention & control to individual and community as part of discharge health education. KNOWLEDGE, SKILLS AND ABILITIES REQUIRED 1. Abilities to react calmly and effectively in emergency situations. 2. Abilities to communicate effectively, both orally and in writing. 3. Abilities to foster a cooperative work environment. 4. Abilities to develop and present educational programs or workshop. 5. Supervises ward clerk on charging and ward stock usage by clients. 6. Knowledges in ward stock (disposable & drugs) and inventory checking. 7. Knowledges of related accreditation and certification requirements. 8. Knowledges of clinical operations and procedures. EDUCATION & TRAINING 1. Attends all mandatory training and ensure that updates are booked in a timely fashion and agreed by the Ward Manager 2. Successfully completes key learning outcomes from the foundation programme during the first month of employment and will complete the main set of competencies within 12 months 3. Initiates own development within agreed parameters. 4. Involves in any aspects of training that need to be cascaded to the whole team such as moving & handling/infection control. 5. Helps induct new staff in ward settings. 6. Assists as mentor/preceptor for trained and untrained staff once appropriate training has been given. 7.
