Pharmacist

**Responsibilities**:

- Registered with Malaysian Board of Pharmacy
- Possess Poison License A and Annual Retention Certificate
- Good Communication skills & interpersonal skills in both English, Malay, Chinese or Tamil.
- Able to work independently & accountability.
- Experience in CDR
- Able to be on-call (after office hour
- Practices within the expressed limits of established policies and procedures, securing approval from the Pharmacy Manager for any deviations from the policies and procedures.
- Ensure the provision of products with optimum quality by following Standard Operating Procedures (SOP) and quality assurance testing (if required).- Participates in the writing, review and amendment of department Policy and Standard Operating Procedures (SOP).
- Maintains statistics and appropriate departmental documents and submits reports as requested by the Pharmacy Manager.
- Liaises with pharmacists or supervisors in other units and Pharmacy Manager in day-to-day issues. Reports to the Pharmacy Manager on a regular basis.
- Liaise with physicians, nurses and other healthcare professionals to ensure the delivery of safe and effective drug treatment.

Review prescription orders and ensures that medications prescribed are safe and appropriate for intended use. This includes potential drug side effects and interactions with other drugs and the suitability of treatments for patient with particular health conditions.- Verification of EMR to ensure that medications prescribed are safe and appropriate for intended use.
- Uses knowledge to advise patients about the safe and appropriate use of medicines they have been prescribed with. Provides counselling on medication/medical device use to patient e.g. method of administration/use, effects, dosage regimen, particularly those who require complex drug therapy as well as provides bedside dispensing and counselling.
- Contacts physicians to clarify any discrepancies or issues that appear in the prescription.
- Ensure the medications reach the patient in the correct form and dose.

Works collaboratively with other healthcare professionals to devise the most appropriate drug treatments for patients.
