**Responsibilities**:

- Practices according to the Pharmacy Department’s and hospital’s policies and procedures. Approval from the pharmacist or supervisor shall be obtained for any deviations from the policies and procedures.
- Ensure the provision of products with optimum quality by following Standard Operating Procedures (SOP) and quality assurance testing (if required).
- Reviews prescription orders or processes ward requisitions and entering them into the Hospital Information System (HIS) to maintain accurate patient medication profiles and inventory records.
- Verification of EMR to ensure that medications prescribed are safe and appropriate for intended use.
- Prepares, labels and dispenses/issues drugs or pharmacy stocks according to Pharmacy Department or hospital SOP.
- To contact the doctor to clarify any discrepancies or issues that appears in the prescription.
- Performs or assists in preparation of pre-packed drugs.
- To compound extemporaneous drug preparations as per formulary, under the supervision of the pharmacist.
- Provides standard instructions or counselling to patients upon issuing their medications as well as provides bedside dispensing. It may refer to the pharmacist if additional information is required.
- Ensure that medicinal products are stored appropriately (according to the manufacturer’s recommendations) and securely to ensure freshness and potency.
- Ensure that medications reach the patient in the correct form and dose.
- Performs checks on the stock level in the pharmacy, wards or units and restocks according to the pharmacy SOP. Informs the pharmacist-in-charge or supervisor of unavailability of drugs.
- Performs regular checks on the expiry dates of drugs or stocks and ensures that drugs or stocks with shorter expiry dates are dispensed or used first. Removes expired drugs from the shelves in a timely manner.
- Keeps work area clean and orderly to maintain the professional appearance of the pharmacy.
- Promotes and maintains quality in all activities and ensures continuous quality improvement measures are followed.
- Counts and sorts out prescriptions. Ensures that prescriptions are compiled, labelled and stored for easy retrieval for at least 2 years.
- Participates in training of pharmacy staff, trainees and other healthcare professionals as required by management (internal and external pharmacy related CPD programs).
- Participates in Pharmacy Departmental meetings and other department or hospital committee meetings and quality improvement activities.
- Participates in preparing infographics under supervision of Pharmacist.
- Strictly maintains patient confidentiality.
- Maintain the physical and microbial parameters of cleanroom and performs Cytotoxic Drug Reconstitution (CDR) under supervision of a pharmacist.
- Obtains and maintains competency in cytotoxic drug reconstitution, under the supervision of the pharmacist. Recertification by the hospital’s Credentialing and Privileging Committee is required every 3 years.
- Performs any other duties as directed by the Pharmacist or Pharmacy Supervisor.**Benefits**:
