Clinical Pharmacist

Clinical pharmacist is responsible to perform various functions through collaborative drug therapy

management and patient care including day-to-day operations and other clinical services related.

**STANDARD PERFORMANCE**:
1. To develop clinical pharmacy programs according to policies & procedures.

2. Ability to evaluate the appropriateness of medications therapy and dosage accordance to patient's diagnosis.

3. To identify related problems and refer to appropriate physicians.

4 Collaboration with other healthcare professionals to ensure optimal pharmaceutical care.

5. Keep proper documentation of medications plans, patient progress and results of pharmaceutical treatment.

6. Demonstrate consistency with Avisena's mission and values.

7. Commit to fully comply with Avisena's Standard professional code of conduct and Organizational

Policies and Procedures.

8. Work effectively with others as a good team player to achieve outcomes.

9. Contribute to a continuous learning environment and innovation to the organization.

10. To take reasonable care at workplace for the safety of oneself, others and to comply with any

instruction or measurement on occupational safety, health and environment as required. Reports

all accidents/ incidents and hazard to the superior and complete the Incident Report Form for each

accident/incident.

11. To perform other related duties as assigned and directed by the immediate superior

**DUTIES & RESPONSIBILITIES**:
1. To develop and run clinical pharmacy programs according to SOP and organization needs.

2. To review patient's record and determine optimum pharmaceutical care according to diagnosis and other related health issue.

3. Develop effective medication plans and discuss any identified issue with the prescribers.

4. To conduct huddle and meeting among the team to focus on care coordination, facilitate immediate face-to-face clarification of issues, result in fewer interruptions during the rest of the workday, and foster a culture of empowerment and collaboration among teams

5. To monitor the team consistently delivering great end-to-end customer experience across stages, channels and touchpoints.

6. To ensure services are provided promptly to patients in an efficient ethical and professional manner.

7. To provide drug information to the medical staff and patients whenever required either by personal contact, issuance of pharmacy bulletins, attendance at meeting, or through participation in various committee meetings.

8. To ensure regular drug checks are done to maintain the quality of drugs kept at all outlets in the hospital.

9. To ascertain that all drugs are stores and handled in accordance to specifications in the hospital.

10. To have direct control in the management of Dangerous Drugs and psychotic substance

11. To conduct and evaluate staff orientation, in-service training and educational programs to ensure that professional pharmacy work practices are carried out all the time.

12. To have verbal and numerical ability required to understand composition, measures and properties of drugs to relate to doctors and medical personnel.

13. To adhere to all Acts, Laws, Rules & Regulations governing the pharmacy services at all the times.

14. To liaise and coordinate with doctors and other departments on issues related to Pharmaceutical Care.

15. To plan staffing requirements & supervise all subordinates and coordinate their activities.

16. To perform any other duties as assigned by designated by the organization.

**Salary**: RM6,500.00 - RM8,000.00 per month
