**Staff Nurse with Post Basic**

**Roles**

Staff Nurse with Post Basic involves both preventing disease and assessing the health needs of patients who are experiencing the real or threatened impact of acute or chronic kidney disease.

Therefore Staff Nurses with Post Basic must be well educated, highly skilled, and motivated. These nurses also deal with every organ system in the body, calling for a holistic approach to patient care that is both challenging and rewarding.

**RESPONSIBILITIES**:
The following duties and responsibilities generally reflect the expectations of this position but are not intended to be all inclusive.

1)Supervises and performs direct patient care including but not limited to:
a. Taking and monitoring vital signs, initiation and termination of dialysis
- i. e.g. to perform procedures such as dressing on the IJC before beginning dialysis,_
- ii. to perform the procedure "Sequential ultrafiltration”,_
- iii. to perform "tight Heparin" procedures,_
- iv. to use the ' Dialyser ' and "Priming" recycling procedure._

b. documentation,

c. physical assessment of patients and patient treatments,

d. taking and transcription of physician’s orders,

e. drawing and documentation of ordered laboratory work.( Send out laboratory tests and radiography and consequential tests)

2) Set ups and monitors dialysis machines and reverses osmosis (“RO”) water system for correct functioning Perform an easy-to-do calibrations of the haemodialysis machine.

3) Notes signs of trauma, patients’ general physical and mental condition, reviews dialysis orders, documents observations in patient medical records.

4) Analyses patient assessments, laboratory data and, when concerns are noted, brings issues to the attention of the physician.

5) Communicates all emergencies, patient life-threatening complications and related incidents Medical Director, Quality Care Nurse, Operations Manager Administrator.

6) Administers patient quality care in a considerate, respectful manner.

7) Handles the record Book of the haemodialysis machines care in the updated state.

8) Visually inspect reused dialyzer and check for the following:
a. Confirm that the level (volume) of sterility (Renalin) in the dialyzer is sufficient

b. Confirm that the blood and dialysate ports on the dialyzer are capped and not leaking. c. Check that the dialyzer is not damaged or leaking. d. Confirm that both the inside and outside of the dialyzer look clean

9) Responsible for the Dialyzer Reprocessing

10) To update inventories of equipment and linen.

11) To control the performance of the standard of haemodialysis and the devices used in the treatment of haemodialysis

12) Monitor the cleanliness of units.

13) Instructs patients on pre
- and post-dialysis care.

14) Provides dialysis therapy, including but not limited to formulating dialysis plan per physician orders, weight and rate of ultrafiltration, verifying safety measures are in place and complied with, administering medications or other appropriate treatments, discontinuing dialysis according to established procedure, documenting all activity appropriately.

15) Assesses patient and family learning readiness and provides appropriate information on renal disease, prescribed dialysis treatment, and medical regimen and its impact on the patient’s state of health and wellbeing.

16) Participates in the development and implementation of the patient’s short and long term plan of care and monthly progress note.

**Minimum Qualifications**
- Education, licenses, certifications, and experience required to fulfil the essential duties, include computer skills as required.
- Nursing diploma/degree from accredited school of nursing required
- New graduate, less than one year after obtaining license
- Current CPR certification required
- Completes Post Basic Renal Nursing Programme from accredited school.

All Staff Nurses with Post Basic Must Have A Common Knowledge Base Relevant To All Aspects Of Care For Adult And Paediatric Kidney Patients And Their Families. This Knowledge Base Includes But Is Not Limited To:

- Anatomy, Physiology, And Pathophysiology
- The Nursing Process As Applied To Nephrology Nursing
- Knowledge Of The Diagnosis And Treatment For Patients Requiring Any Method Of Renal Replacement Therapy
- Pharmacology And Pharmacotherapy
- Nutrition
- Growth And Development
- Teaching/Learning Theory
- Counselling/Interviewing Skills

**Work Environment**

Job required tasks routinely involve a potential for mucous membrane or skin contact with blood, body fluids, tissues or potential spills or splashes.

Uses of appropriate personal protection measures are required for every healthcare provider in these positions.

Care may be extremely complex: patients may have numerous comorbid conditions including, but not limited to, cardiovascular disease, diabetes, hypertension, infectious disease, bone disease, or psychiatric conditions.
