1. Manage and organize the Health Records Service.

2. Ensure policies and procedures are maintained. Updated and carries out by the staff

3. Ensure documentation on the Medical Record is compliant to MOH standards.

4. Ensure timeliness in statistical reporting.

5. Assist in Data Analysis.

6. Maintain confidentiality and security of patients’ Medical Records.

7. Prepare Annual Budget for the department.

8. Prepare Strategically Plan for the department.

9. Audit Patient Medical Records.

10. Maintain stock and stationery inventory

11. To code diagnoses from Ward Discharges using ICD-10

12. To code medical and surgical procedures using PHFSR

13. To monitor delinquent Medical Records by doctors and remind the doctor to complete the diagnoses and discharge summaries

14. To process and send the monthly E-reporting to the Ministry of Health before the 22nd of the following month.

15. Ensure the safekeeping of all records with the department.

16. Ensure timelines in submitting reports to relevant.

17. Maintain confidentiality of medical Records.

18. Any other task been assigned by the superior.

**Salary**: From RM1,500.00 per month
