Speech Therapist

1. To be professional and legally responsible and accountable for all aspects of your own work including the management of patients in your care. To ensure a high standard of clinical care for the patients under your management.

2. To undertake the comprehensive assessment of patients, including those with a complex presentation, using investigative and analytical skills and to formulate individualized management and treatment plans, using clinical reasoning and utilizing a wide range or treatment skills and options to formulate a specialized programmed of care.

3. To represent ReGen Rehab externally (locally, regionally or nationally) regarding speech and language therapy services provided to patient.

4. To interpret and analyze clinical and non-clinical facts to form accurate diagnosis and prognoses in a wide range of highly complex conditions, to recommend the best course of intervention and to develop comprehensive discharge note.

5. Establish treatment goals and needs for each patient and their capabilities.

6. Meets the patient’s goals and needs by providing high quality care by assessing patient’s speech, language, communication problem and swallowing abilities and interpreting evaluations and test results; determining speech therapy treatment plans in consultation with physicians or by prescription. Monitor patient’s progress, re-establish goals and revise treatment plan as necessary. Alert physician to changes in patient’s condition.

7. For patient with little or no speech capability, promotes maximum independence by selecting and constructing augmentative or alternative communication method, including automated devices and sign language and therapies according to individual physical capacity, intelligence level and interest.

8. Document patient care services by charting in patient and department record including writing diagnostic reports, progress reports and re-evaluation reports.

9. Review patient’s chart, interview patient/family and consults with physicians, rehab therapist and other relevant clinical professionals in order to evaluate patient condition according to standard procedures.

10. Counsel patient, family members regarding communication deficits and instructs them in proper management techniques.

11. Completes discharge planning by consulting with physicians, therapists, teachers and other health care workers contributing to patient care conferences.

12. Assures continuation of therapeutic plan following discharge by designing and instructing patients, families, and caregivers in home exercise programs, recommending and/or providing assistive equipment, recommending outpatient or home health follow up programs.

13. To accept clinical responsibility for a designated case load (KPI) of patients, and to organize this effectively and efficiently with regard to clinical priorities and use of time.

14. To demonstrate highly developed, coordination and sense for assessment and manual treatment of patients.

15. The regularly participate in working parties developing policing and service development changes.

16. To work with the Therapy Director/Manager in developing the strategic and operational management of the therapy services.

17. To provide spontaneous and planned advice, teaching and instruction to relatives, and other professionals to provide understanding of the aims of speech and language therapy treatment, and to ensure a consistent approach to patient care.

18. To communicate effectively with patients and cares to maximize rehabilitation potential and to ensure understanding of condition.

19. To protect patients and employees by adhering to infection-control policies and protocols.
