**Responsibilities**:
**Patient Assistance**
- Assist patients with activities of daily living, including bathing, dressing, toileting, and mobility.
- Provide support and companionship to patients, promoting a compassionate and patient-centred care environment.

**Vital Signs Monitoring**:

- Take and record patients' vital signs, including temperature, pulse, blood pressure, and respiratory rate.
- Report any abnormal findings to the nursing team for further assessment.

**Patient Transfers**:

- Assist with patient transfers, repositioning, and mobilization in accordance with established care plans.
- Ensure the safety and comfort of patients during transfers.

**Assistance with Procedures**:

- Assist nurses with various medical procedures, including wound care, dressing changes, and specimen collection.
- Provide support during examinations and treatments as directed by the nursing team.

**Environmental Care**:

- Maintain a clean and safe patient environment, including changing bed linens, cleaning bedside tables, and ensuring overall tidiness.
- Adhere to infection control practices and contribute to a hygienic care setting.

**Patient Observation**:

- Observe and report any changes in the patient's conditions or behaviours to the nursing team.
- Communicate effectively with nursing staff regarding patient needs and concerns.

**Equipment Handling**:

- Handle and operate patient care equipment, such as bed controls, lifts, and monitoring devices.
- Report any malfunctioning equipment to the appropriate personnel.

**Collaboration with Healthcare Team**:

- Work collaboratively with nurses, physicians, and other healthcare professionals to ensure coordinated and effective patient care.
- Participate in team meetings and communicate relevant information to the nursing team.

**Documentation**:

- Complete accurate and timely documentation of patient care activities, including vital signs, patient assistance, and observations.
- Maintain confidentiality of patient information.

**General Responsibilities**:

- Obtains reports from the previous shift's staff, focusing primarily on patient and department related issues.
- Contributes to the achievement of clinical quality outcome indicators including patient satisfaction.
- Upholds and applies the organizational values to provide quality patient care.
- Provides direct assistance to patients as needed in meeting their personal hygiene needs, including but not limited to, bed baths /sponging /showering, changing patient’s clothing and oral hygiene.
- Attends to patient’s toiletry needs, providing bedpan, urinal or assistance to and from the bathroom as required.
- Assists in the lifting, movement and handling of patients including positioning and re-positioning of bedridden patients and the mobilization of patients.
- Assists patients with eating, and drinking. Assists with feeding patient (during and in between mealtime nourishment including fluids when required).
- Assists the nursing staff with the delivery of basic patient care including maintaining patients’ personal hygiene and toileting.
- Provides comfort measures such as pressure area care, back care, head, foot and hand massage as appropriately needed.
- Ambulates patient and sits patient out of bed and vice versa together with the nursing staff.
- Assists in the maintenance of a clean and tidy environment in the workplace and patient’s room including the proper disposal of waste.

- Monitors patient activities and reports all patient-related information to the staff nurse correctly, appropriately and in a timely manner including unusual occurrences or reactions.
- Maintains close visual observations of restless patients.
- Measures and records the readings of temperature, blood pressure, pulse and respiration rates, oxygen saturation and food and fluid intake and output, height and weight as directed and inform the staff nurse of any abnormal recordings.
- Accurately measures, empties and documents the content of urinary catheters and drains.
- Documents all relevant information correctly, appropriately and in a timely manner.
- Assists nursing staff during the transfer of patients from one ward/unit to another. Position / hold the patient during dressings and other procedures when required.
- Receives patient for admission and orientates the patient to the room/unit and services.
- Prepares patients for procedures as directed, assists the physician and remains with the patient if required.
- Collects specimens such as urine, faeces or sputum and despatch to the lab as directed.
