Job Description

  1. Patient Assessment & Clinical Coordination

  2. Conduct initial nursing assessments for patients referred to the Weight Management Clinic, including medical history, lifestyle habits, and readiness for change.

  3. Coordinate multidisciplinary consultations (Internal Medicine, Endocrinology, Dietetics, Psychology, Physiotherapy, Gastroenterology, and Bariatric Surgery).
  4. Develop individualized weight management care plans in collaboration with the multidisciplinary team.
  5. Monitor patients' clinical progress, adherence to treatment, and response to interventions.

  6. Assist in preoperative and postoperative clinical assessments for bariatric surgery candidates

  7. Patient Education & Support

  8. Provide evidence-based counselling on nutrition, physical activity, behavioral change, and medication compliance.

  9. Educate patients and families on weight-related comorbidities such as diabetes, hypertension, sleep apnea, and metabolic syndrome.
  10. Facilitate group education sessions, health talks, and lifestyle workshops.

  11. Offer continuous motivational support and counselling to encourage adherence and long-term behavioral change.

  12. Care Navigation & Coordination

  13. Serve as the main point of contact for patients throughout their weight management journey.

  14. Schedule appointments and follow-ups across multiple specialties and allied health teams.
  15. Coordinate pre-admission, pre-surgical preparation, and postoperative follow-up for bariatric surgery patients.
  16. Ensure clear communication among care teams to prevent duplication, delay, or clinical error.

Job Requirement

  1. Recognized Diploma / Bachelor of Nursing from an accredited University.
  2. Valid Annual Practicing Certificate.
  3. Minimum 7 years of working experience in clinical work in healthcare industry.
  4. Good understanding of multidisciplinary obesity management and surgical care coordination.
  5. Good experience in patient counselling, case coordination or health education is highly desirable.
  6. Good verbal and written communication skills.
